Mandatory forms for childcare service
The parent handbook is meant to be used as a reference and should answer most of your questions about our policies and procedures. The last page of the parent handbook, the “Child care agreement” form, must be returned when registering your child.
An about me form that is specific for infants entering child care. It is to be filled out by the parent to help the teachers and staff get to know your family and child better. Please be as detailed as possible, as more information helps us make your child’s first experience with us comfortable and enjoyable.
This registration form is for all children and is required by the Bureau of Child Care Licensing. These forms must be updated annually.
All children are required to provide a health form with documentation of immunizations to be enrolled in care. This is a form that you can bring to your child’s doctor’s office but if they provide one that has the same pertinent information we can accept that form as well.
